[chef] Check constraint error when creating new user accounts for LDAP users

I've configured my Chef server to use LDAP-based authentication. When I log in
with a new user I reach the "Link Account" page. When I select "I don't have a
Chef account yet" and click "Create new account", the chef server logs the
following error:

INFO: HTTP Request Returned 500 Internal Server Error: PG::CheckViolation:
ERROR:  new row for relation "users" violates check constraint
"password_hash_deps"

And the web interface displayes the following error message:

An application error has occurred. Please try again later. (Error 10000)

Creating users manually and linking LDAP accounts to existing users works 
fine.
Is this a bug, or a misconfiguration on my part?

I'm using Private Enterprise Chef v11.1.2 on CentOS 6.5
