Eric, you can also install the Chef 12 premium features (opscode-manager),
and as long as you have <= 25 nodes you should be ok.

Now that I know what to look for, I found
http://docs.getchef.com/migrate_to_hosted.html. This also describes the
updated chef_server_url. Perhaps the new Chef Server 12 documentation should
make clear that for those who had been using open source Chef prior to chef
server 12, that they must follow the instructions for upgrading to Enterprise
Chef (since, Chef Server 12 incorporates the previous Enterprise Chef
functionality.
