Which region are you trying to launch instances in? What happens if you try a different region? Can you increase the timeouts on the knife calls?

Waiting for server...ERROR: Fog::Errors::Error: Reload failed,

Fog::Compute::AWS::Server <instance id here> went away

Any suggestions? Totally at an end with this thing....
